Job Description

Position 6 Month Fixed Term Contract/Internal Secondment – Patient Safety Investigator and Inquest Assistant (Trust Patient Safety Incident Team). Role Overview As a Patient Safety Investigator and Inquest Assistant you will support daily management of patient safety incidents, conduct learning reviews, coordinate investigations and provide training, in line with the Patient Safety Incident Response Framework (PSIRF) and Systems Engineering Initiative for Patient Safety (SEIPS). Key Responsibilities Support daily management of patient safety incidents within the Trust in line with PSIRF. Conduct thorough initial response reviews to help determine appropriate response. Ensure Service Users, Carers/Families, Trust Staff, and third parties are fully involved in the response process. Coordinate a variety of investigation responses, including Patient Safety Incident Investigations (PSII’s), Case Reviews and After-Action Reviews. Coordinate a programme of embedded learning events focusing on themes arising from incidents. Support and prepare witnesses for inquest, attend inquest as a constant attendee and record when required. Deliver PSIRF and SEIPS training to new staff (band 7 and above) who may be on a patient safety incident response panel. Provide expert advice to all staff on matters related to patient safety incidents and investigations, ensuring compliance with local and national guidance. Provide training to staff for undertaking investigations, supporting all affected parties throughout the process. Ensure compliance with all statutory reporting requirements. Plan, organise and conduct investigations into patient safety incidents, adjusting plans or strategies as necessary. Work autonomously as an investigator, escalating risks to the Patient Safety Incident Team Lead. Identify and review all sources of information in relation to incidents, ensuring appropriate response. Effectively interview staff and service users in difficult circumstances to establish facts, including reluctant witnesses. Provide specialist advice in investigations and SEIPS to improve care and treatment. Receive and provide highly complex, sensitive and contentious information relating to incidents and present to teams as appropriate. Liaise with service users, carers, victims and their families throughout the investigation, encouraging engagement. Advise the Team Lead on the quality, suitability and effectiveness of completed reports. Present complex information to internal and external stakeholders, including senior clinicians and front line professionals. Critically evaluate and analyse diverse evidence from multiple sources. Recommend whether individuals involved in incidents should be dealt with through other processes under the “Just Culture” principles. Monitor progress of investigations and implementation of recommendations, providing exception reporting. Produce, develop and present high‑quality reports and analysis on incidents to the Trust Board and required committees. Review safety action plans, identify trends and themes, and ensure learning is embedded into practice. Undertake audits, assurance visits and debriefs to discuss learning and review action implementation. Participate in the development of an effective team and productive working relationships across the Trust. Remain impartial and objective at all times. Advise, encourage and share knowledge using latest research and practice development. Apply principles of blame‑free investigation across diverse clinical services and recognise when specialist expert opinion is required. Contribute to development and improvement of systems and processes for investigations and lead allocated improvement projects. Review medical records to identify relevant staff for coronial requests. Support staff involved in coronial investigations and inquests, assisting with statement preparation for HM Coroner. Required Experience & Skills Experience conducting investigations under the Patient Safety Incident Response Framework. Understanding of the Systems Engineering Initiative for Patient Safety Framework. Passion for supporting the patient safety and patient experience agenda. Experience with Datix or similar incident reporting system. Experience in conducting learning reviews such as Response Reviews, Case Reviews and After Action Reviews. Capacity to work independently and collaboratively with multidisciplinary teams. Strong communication and interviewing skills, including handling sensitive and difficult situations. Knowledge of statutory reporting requirements and “Just Culture” principles. Ability to produce clear, complex reports and present findings to senior leadership. Evidence of training delivery and mentoring staff on investigations and SEIPS.
